           The proposed funding will support scientific research and innovation in cancer care within an applied physics program run jointly by Mary Bird Perkins Cancer Center and Louisiana State University. Since 2004, MBPCC and LSU have operated an integrated training and research program in the field of medical physics, which is an applied branch of physics concerned with the application of the concepts and methods of physics to the diagnosis and treatment of human disease. In the setting of cancer therapy, medical physicists develop new therapeutic techniques, collaborate with the treating physician (radiation oncologists) to design individualized treatment plans, and monitor equipment and procedures that ensure cancer patients receive the correct dose of radiation to the correct location. Due to the highly specialized nature of the field, a career in medical physics requires an undergraduate degree in physics, a graduate degree in medical physics, and postgraduate residency training in medical physics.

Graduate students enrolled in MBPCC-LSU medical physics program, one of only 40 accredited such programs in the country, receive classroom and research training in their pursuit of Masters and/or Doctoral degrees in medical physics. After completing their graduate training, these professionals then enter residency training programs in preparation for the board certification exam and, ultimately, a career in cancer care. In that regard, MBPCC also houses the nation’s largest accredited residency program in medical physics to provide the complete continuum of training (classroom, research, and clinical) in medical physics. Our program serves as a national model for a successful public-private partnership because it leverages the resources of a research university to improve cancer care in the community setting. As a result, the program attracts top candidates (both students and faculty) from around the country.

Cancer patients in Louisiana benefit from our program primarily through the development of novel treatment technologies and techniques that improve survivorship, lessen side effects, and increase the efficiency of care delivery in our community. The size and regional presence of MBPCC means that innovative technologies are rapidly distributed across a network of facilities within southeast Louisiana. The academic culture of the program encourages publication of research in peer-reviewed scientific journals, further proliferating advances made in Louisiana to locations throughout the world. As evidence of this, the program has published more than 40 research papers in peer-reviewed journals over the last four years.

Cancer therapy is highly individualized, technologically intensive, and extremely complex. Each patient has a treatment plan that is tailored to his or her disease and anatomy. The customization of each plan is achieved through a complex chain of events that includes imaging, computer modeling, virtual treatment comparisons, and sophisticated treatment delivery technology. In particular, the computer modeling process is labor intensive and subjective. More complex cases can require several days for the medical physicist, treating physician, and other members of the treatment team to collaborate and determine the ideal plan of treatment. Much of this time on the front end of the treatment planning process is done via computer modeling whereby medical physicists conduct a lengthy process of making many dose adjustments and assessing the resulting changes in the distribution of dose throughout the body. This process is necessary to develop the “ideal” and highly tailored treatment plan.

          The purpose of our requested project is to develop a software tool that can be used by the treatment team to estimate the “ideal” dose distribution for each patient before beginning the time and labor intensive computer modeling process. Such a project would have two major benefits: First, the medical physicist(s) would already have an estimate of what could be achieved in terms of distribution doses, thus significantly reducing the time required to develop a treatment plan. This would, in turn, reduce the time and resources needed to develop the plan and facilitate more rapid treatment for patients, many of whom require treatment to begin as quickly as possible. Secondly, but most importantly, this research would further enhance the quality of the treatment plan because the subjectivity associated with developing an ideal plan would be removed. This approach would provide for more consistent solutions for patients. Having an estimate of the ideal plan would allow the medical physicist to immediately navigate to the best possible treatment plan.

What are the goals and objectives for achieving such purpose?
          The proposed project will utilize a set of patients that will serve as library or reference cases. Ideal plans will be developed for each using a plan optimization utility available on our dedicated research treatment planning computers. The utility is too slow to be used routinely in the clinic, but is a perfect solution for generating the proposed library of “ideal” plans. A mathematical relationship between the qualities of the ideal plan and the anatomy of the patient will be created. This relationship will then be applied to future patients by determining which library case the current patient most resembles and selecting the corresponding plan as the “ideal” one for the patient. The ideal plans will be compared with those that are constructed using the trial-and-error approach in order the test the effectiveness of the technique. Development and testing of the tool will be begin with prostate cancer patients, and then extended to other sites such as breast, lung, and brain once feasibility is established.
The requested funding for this project is $75,000 for one year. The request funding would provide one year of stipend support for a MS student ($20,000) and PhD student ($30,000) enrolled in the MBPCC-LSU graduate program in medical physicist. Approximately 10% effort of a staff physicist at MBPCC ($25,000) is also required to lead the project and supervise the graduate students. Additional resources needed for the project – such as the treatment planning software, analysis software, and desktop computers – will be provided by MBPCC. Once feasibility is established, further support for the PhD student and staff time may be requested in future NGOs.
