1. Supporting Youth, Families, and Maternal and Child Health
Family Road of Greater Baton Rouge is a 501(c)(3) nonprofit with 27 years of experience delivering direct services and coordinating care across a network of more than 108 nonprofit, private, public, and government partners. We provide a comprehensive “one-stop” hub for families, addressing health, social, and economic needs through seven structured service areas: Healthy Pregnancy & Healthy Baby, Parenting, Car Seat & Home Safety, Banking & Technology, Counseling & Wellness, Education, and Support Services.
Our services are offered in person and virtually, including free pregnancy testing, sonograms, and support services. We also serve as a Gold Star Application Center for DCFS and Medicaid. At the core of our maternal and child health focus is the Healthy Start Program, which, for 24 consecutive years, has supported over 700 pregnant women, post-partum, inter-conception women, infants, and their partners annually. This program features case management, home visits, clinician access, health education, and parenting classes. Family Road understands that education and services are vital to a community’s success. Education is provided to the community, including schools, religious organizations, and group homes. Family Road delivers a curriculum based on abstinence only and abstinence plus. Both focus on the prevention of pregnancy, STDs, HIV/AIDS, goal setting, and building self-esteem with abstinence. Parental consent is obtained for programs, and parents are also offered programming on health and support in creating a healthy parent-child relationship. In addition, Family Road also provides parenting classes with a 5–7-week series developed from the evidence-based program Nurturing Parents, where parents gain knowledge and education on nurturing parenting skills, discipline, and accountability.
2. Scope of Work- With grant funding, Family Road will enhance outcomes for mothers, fathers, babies, and whole families by delivering comprehensive, wrap-around maternal and child health services throughout Greater Baton Rouge. We will proactively engage expecting and parenting mothers and fathers, as well as youth, through a multidimensional care model that includes: Clinical services led by our maternal and child health clinician, offering free pregnancy testing, prenatal classes, centering pregnancy groups, support groups for new parents, and mental and physical health screenings, Health education sessions, including abstinence and parenting classes, are delivered by our trained facilitators, Home visitation and outreach by case managers and licensed behavioral health clinicians, serving families prenatally and into the postpartum period, Educational and screening materials are distributed across the region to support access and learning. Funds will also support collaboration with our Greater Baton Rouge Maternal & Child Health Consortium and local clinical and social service providers. Together, we will deliver training on maternal mental health and develop a comprehensive social service resource map for the region. Additionally, we will launch a robust "Hear Her Concerns" social media and bulletin campaign to raise awareness of maternal health risk factors. The campaign empowers pregnant and postpartum individuals to voice warning signs and urges their support networks to listen carefully. Our campaign will emphasize timely recognition and response to maternal and infant health risks. This multi-pronged strategy is grounded in evidence demonstrating that wraparound support models significantly improve outcomes, including higher exclusive breastfeeding rates, better birth weights, and reduced mental health complications. By increasing community awareness, expanding service delivery, and building clinical and social capacity, Family Road will elevate the reach and quality of maternal and child health care, helping us meet and exceed community needs.

Program Goals:
Increase awareness and access to services for maternal and child health to women and their families in the Greater Baton Rouge Community through social media, print and broadcast media campaigns, and services such as case management, clinical services, and mental health counseling.
