        Hyssop Care Foundation provides temporary emergency shelter resource services, educational resource tools and basic care necessities to at-risk youth who are transitioning from homeless to self-sufficiency as young adults. Our target populations are youth experiencing homelessness between the ages of 16 to 21 experiencing homelessness and young adults who have left foster care after the age of 18 up to age 21, but may need alternative housing and services. We currently serve two (2) parishes within the State of Louisiana which are Caddo and Bossier.
Hyssop Care Foundation are requesting funding for our Transitional Living program specifically youth between the ages of 16 years old to 21 years old who have left foster care after and the age of 18 up to age 21, but may need alternative housing and services.
There is a true need in the North Louisiana area for a complete Transitional Living Outreach program. Studies have shown that one in seven youth between the ages of 10 and 18 will run away from home or a foster home and youth between the ages of 12 and 17 are more at risk of homelessness than adults. 75% of those runaways are female and 75% of runaway youth have dropped out or will drop out of school. There are no existing transitional living programs within Region 7 specifically targeting this demographic of homeless or foster care youth in North Louisiana.

                  Our proposed program will operate a transitional living center, an undisclosed safe house and a team to travel the community to hand out basic care items such as clean clothing, blankets/jackets (weather appropriate), food, personal hygiene products. With funding, the center can also serve as a safe house that will provide the same base care items along with education resource services, job readiness training as well as counseling/group therapy. Our safe house will be a facility where a youth can receive a hygiene kit and/or shower, participate in counseling/group therapy, apply for jobs, apply and attend school online, receive temporary shelter and receive a meal. Our goal is to prepare this population for self-sufficiency and providing education resources services, counseling, survival aid intervention and prevention services to youth who are at risk of homelessness, abuse and sexual exploitation.
Through funding from the state, we will be able to provide a needed service to the community while laying a foundation that will not only rebuild the lives but also plant a seed for long-term stability for individuals who fall victim to food insecurities, exploitation and abuse.
It’s not always easy to identify individuals on the streets through typical counts of unsheltered people experiencing homelessness. They are often less willing to disclose that they’re experiencing homelessness or may not even identify as homeless. They also may work harder to try to blend in with others who aren’t homeless. 
Hyssop Care Foundation, Inc. will implement the following key components for participants in our Transitional Living Program:
1.	Improve the crisis response.
2.	Prioritize family reunification or support as the initial intervention.
3.	Expand the reach and effectiveness of transitional life skills programs.
4.	Improve data collection and performance measurement as required by RHYMIS/HMIS Federal Performance Measures and Standard Data Collections.
5.	Collaborate with mainstream systems such as transportation services, mental health evaluation programs, child welfare and juvenile justice.
        
        What are the goals and objectives for achieving such purpose?
                  Given the importance of making steady, incremental progress towards reducing homelessness and statistics in North Louisiana, it is unlikely that insurmountable results will be attained within the first year. During the 2025-2026 Fiscal Year, emphasis will be placed on implementing and maintaining the program structure; outcome measures, evaluating the program growth and intensive mental health counseling as well as transitional life skills for participants.
The following are the program outcomes expected the first fiscal year:
¿	To provide temporary shelter or safe house transitional living for 50% of participants entering our program.
¿	To have 50% of participants in our program successfully enrolled in a local parish school or GED Program.
¿	To have 50% of participants in our program successfully complete a local GED Program.
¿	To maintain of 50% success rate for the participants who have completed our program (determined by family reconnection, stable mental health, education goals being met, job training, stable employment, and stable living condition.)
