Provide a summary of the project or program:
Split Second Foundation is a New Orleans based nonprofit healthcare organization dedicated to improving the lives of aging adults and people with disabilities. Our mission is to provide comprehensive care, education, resources, and advocacy that help individuals live healthier, more independent, and more connected lives.
We were founded from lived experience and built on the belief that disability and aging should never mean limited access to wellness, opportunity, or quality support. Through our work, we address gaps that traditional systems often leave behind by offering a more holistic, person centered approach to care.
Split Second Foundation serves individuals through health and wellness programming that supports the physical, mental, social, and practical needs of the people we work with. Our services include adaptive fitness and personal training, wellness programming, support services, community education, and resource navigation. We also create spaces where individuals and families can find connection, encouragement, and tools to better manage life after injury, illness, disability, or age related challenges.
In addition to direct services, the Foundation is working to expand its impact through Split Second Health and Wellness, an integrated model designed to bring clinical care and wellness services together under one roof. This vision includes therapy, counseling, case management, nutrition support, and fitness, creating a more complete continuum of care for the communities we serve.
At its core, Split Second Foundation exists to help people not just survive, but thrive. We are building a model that promotes dignity, access, independence, and long term well being for those too often overlooked by the healthcare system.

What is the entity's public purpose, sought to be achieved through the use of state monies?
The public purpose of Split Second Foundation is to improve the health, independence, and quality of life of aging adults and people with disabilities by expanding access to care, wellness, education, and support services that are often limited or inaccessible through traditional systems. The use of state monies would help advance a clear public benefit: reducing health disparities, improving functional outcomes, strengthening community based supports, and helping vulnerable residents remain healthier, safer, and more engaged in their communities.
Split Second Foundation serves individuals who are frequently at greater risk for poor health outcomes, social isolation, preventable complications, and costly institutional care. Through adaptive fitness, wellness programming, support services, resource navigation, education, and community based engagement, we help people build strength, improve mobility, manage chronic conditions, and increase confidence in daily life. These services are designed not only to address immediate needs, but to create long term stability and better outcomes for individuals and families.
The impact on the community is significant. Our programs help participants improve physical health, mental well being, and overall independence. For many, this means better balance, greater endurance, improved confidence, stronger social connection, and more consistent access to supportive resources. Families also benefit by gaining guidance, relief, and a trusted community partner to help navigate complex challenges related to disability, aging, caregiving, and recovery.
On a broader level, our work helps fill critical gaps in the continuum of care. By providing preventative, supportive, and restorative services in a community setting, Split Second Foundation helps reduce the likelihood of avoidable hospitalizations, setbacks, and long term reliance on higher cost systems of care. State investment in our work supports a model that is proactive rather than reactive, community based rather than institutional, and centered on dignity and access.
Ultimately, the public purpose is to create a healthier, more inclusive Louisiana where aging adults and people with disabilities have the opportunity to live fuller, more independent lives. Funding Split Second Foundation would strengthen a proven community resource that delivers meaningful outcomes for residents while contributing to the long term health and resilience of the broader community.
What are the goals and objectives for achieving such purpose?
The goal of Split Second Foundation is to improve health outcomes, independence, and quality of life for aging adults and people with disabilities by expanding access to comprehensive, community based care and wellness services. Our objective is to meet the physical, mental, social, and practical needs of individuals who are too often underserved by traditional healthcare and support systems.
To achieve this purpose, Split Second Foundation works to provide accessible health and wellness services that help participants build strength, improve mobility, increase confidence, and better manage the challenges associated with disability, aging, injury, and chronic conditions. This includes adaptive fitness, wellness programming, support services, education, and resource navigation designed to help individuals live healthier and more independent lives.
A key objective is to reduce barriers to care. Many of the people we serve face obstacles related to cost, transportation, limited specialized services, or a lack of coordinated support. Our programs are designed to close those gaps by delivering person centered services in a supportive community setting that promotes dignity, consistency, and long term engagement.
Another objective is to strengthen community integration and reduce isolation. Through our services, participants gain not only access to programming, but also connection, encouragement, and a greater sense of belonging. Families and caregivers also benefit from guidance, support, and access to trusted resources that help them better navigate complex care needs.
In the long term, Split Second Foundation aims to expand its impact through the development of Split Second Health and Wellness, an integrated model that brings clinical care and wellness services together under one roof. This vision includes therapy, counseling, case management, nutrition support, and fitness, creating a more complete continuum of care for the community.
Ultimately, our goals and objectives are centered on prevention, restoration, and empowerment. By helping people improve their health, maintain independence, and access needed support earlier, we seek to create better individual outcomes, stronger families, and a healthier, more inclusive community.
